We offer personalized guidance, including step-by-step instructions and language support, to facilitate access to these vital services. This initiative reflects our commitment to removing barriers and ensuring that Black New Jerseyans with I/DD receive the support they deserve, empowering families to navigate the system confidently.

During the first year of our grant project, data collected from consortium meetings, feedback from families, and insights from participants in the EPIC program revealed a significant need: Black New Jersey residents with intellectual and developmental disabilities (I/DD) and their families often struggle to navigate the PerformCare application process to access critical support services. This discovery led us to develop a program designed to help families successfully navigate PerformCare’s system and access the resources they need.

How We Help

During the first year of our grant project, data collected from Consortium meetings, feedback from families, and insights from participants in the EPIC program revealed a significant need: Black New Jersey residents with intellectual and developmental disabilities (I/DD) and their families often struggle to navigate the PerformCare application process to access critical support services. This discovery led us to develop a program designed to help families successfully navigate PerformCare’s system and access resources.

To address this need, we created a program to provide families with hands-on guidance through the PerformCare application. The program helped families overcome the challenges of application navigation, making it easier to receive support. Our team worked closely with community partners and families to develop this program, ensuring it was as helpful and accessible as possible.

Through small group and one-on-one sessions we offered detailed, easy-to-follow guidance on the application process, language support, access to technology and a step-by-step approach to increase understanding and reduce confusion.

Our goal is simple but powerful: to make applying for PerformCare more accessible to Black NJ residents with I/DD, giving them and their families the support they need to live full and empowered lives.
